Information has to be complete and honest. Each gynaecologist will explain what he does, and
what the alternatives are. Information thus may vary from one gynaecologist to the other.
The information given will reflect the existing guidelines.
It is important to know that guidelines mainly reflect evidence based upon RCT, ie common attitudes to diagnosis and treatment , not necessarily the best available
For surgery solid evidence is rarely available since results reflect largely depend on the skill of the surgeon.
Often several alternatives exist. It is important that the patient knows which choices
are made and why.
For surgery the patient should realise there is no quality control of the individual intervention. Only when the whole surgery is registered,
you can know exactly what has been done. Those who tape systematically can be recognised by the label above.
Some examples.
Hysterectomy can be performed as a vaginal hysterectomy, as an abdominal hysterectomy (with a big incision), as a total laparoscopic
hysterectomy (with 3 minor incisions) or as a subtotal hysterectomy (ie without removing the cervix), while often (eg for bleeding)
hysteroscopic surgery is enough. Duration of hospitalisation will vary from a few hours to 1 day, to several days or 1 week.
Also postoperative pain varies
For deep rectovaginal endometriosis a bowel resection is rarely indicated (less than 1% for the rectum). Conservative surgery requires
however an experienced surgeon.
Information has to reflect the individual gynaecologist. For surgery it should give the personal experience and the
personal results. As a quality control the intervention should be taped. It is recommended to share the video registration with the patient
For medical therapy there is generally less discussion since based upon solid data. Important differences in the interpretation of scientific data exist. A typical example is hormone replacement
therapy after menopause. It is important that you know what your doctor stands for and why.
